Bilingual Family Coach- Metropolitan Family Services

Bilingual Family Coach to join our North Team!

ESSENTIAL JOB FUNCTIONS: 

  • Works with the full family in a collaborative and supportive manner to help, motivate, and inspire individual and family goal-setting while allowing the family to drive the work.
  • Have a deep respect for a family’s expertise, which is fundamental to establishing trust.
  • Help the family to identify strengths and then work with the families to utilize these strengths and build others.
  • Use of trauma informed practices.
  • Establish and maintain accurate documentation of demographic information required by the funder(s) for families in the program.
  • Maintain accurate and current documentation in Smartcare and complete other tracking forms needed to meet program deliverables.
  • Completes the intakes and monitors family goals and activities.
  • Provides follow up to ensure family goals are progressing or achieved.
  • Provide case management services to families and children including assistance and referrals. Documents services provide in all service areas.
  • Works collaboratively with community partners. Develops a knowledge of resources and helps clients to access them.
  • Maintain on-going contact with families regarding program services. And ensure a smooth transition to referrals provided.
  • Coordinates events and activities within the region to meet client needs and engage individuals, seniors, households in services.
  • Offer and provide assistance to families with barriers to accessing services. Provides support services including transportation.
  • Records family progress in SMARTCARE and case files.
  • With consultation of supervisor, handles emergency and crisis situations within the target population.
  • Works within contract, grant, and program requirements including meeting timelines and reporting requirements
  • Utilizes a team approach and works with other program staff in the planning and implementation of services.
  • Takes increasing responsibility for own professional development, utilizing supervision and other developmental opportunities within and outside the agency to enhance skills and understanding.
  • Works effectively with diverse staff and service populations. Accountable for providing exception customer service.
  • Required to work evenings and weekends as needed to meet program needs.
  • Other duties as assigned by supervisor.

QUALIFICATIONS:                               

  • High School Diploma or equivalent with two (2) years of case management experience, which includes the coordination of youth, senior, and family services. Required (Bachelor’s Degree preferred) 
  • Demonstrated ability to render adequate service to clients and to organize and manage job efficiently. 
  • Requires mobility to work with community-based caseload. 
  • Ability to maintain assigned community and home-based services. 
  • Ability to work effectively with diverse individuals and groups.
  • Must be highly organized and knowledge of Excel to track data.
  • The 40-hour domestic violence training from an approved site is preferred.
  • Bilingual in English and Spanish required.
  • Ability to work independently as well as in a team setting.
  • Candidates must have regular access to reliable transportation or a reliable vehicle with insurance.
  • Ability to work a flexible schedule including some evenings and/or weekends if needed.
  • Satisfactorily pass a health examination, fingerprint and criminal background check.
