Bilingual Contact Tracer- Metropolitan Family Services

Full-time Bilingual Contact Tracer to join our North Team!

PROGRAM SUMMARY:

The Chicago Department of Public Health (CDPH) is responding to the emergency presented by COVID-19. During the period of community transmission, CDPH is working continuously to detect new cases, identify sources of transmission, protect vulnerable populations and control spread. These efforts are essential to keep the numbers of affected individuals low enough that our healthcare community can provide all needed services.

The Chicago Department of Public Health selected the Chicago Cook Workforce Partnership to serve as the Lead Coordinating Organization to implement the Chicago COVID Contact Tracing Corps program. The Contact Tracing Corps will consist of a variety of positions including Contact Tracer and Supervising Contact Tracer. The following Project Partners will deliver training, professional development and resource coordination: University of Illinois at Chicago School of Public Health, NORC at the University of Chicago, Malcolm X College of the City Colleges of Chicago, and the Sinai Urban Health Institute.

Contact Tracers must comply with all Chicago Department of Public Health and Chicago COVID Contact Tracing Corps training regarding confidentiality and the handling of personally identifiable information; and follow all scripts, policies and procedures established for the program. 

This position offers full-time with shifts available seven days a week, including evenings and weekends. The position is grant funded for a minimum of 10 months, with a possible extension for up to an additional 12 months (for a possible total of 22 months). Continuation of the position is dependent upon sustained grant funding and public health need. NOTE: Learn-as-you-earn training opportunities to prepare for permanent, full-time placement in public health and related fields following completion of the Contact Tracing Corps are under development.

ESSENTIAL JOB FUNCTIONS:

  • Conduct outreach to individuals to discuss their potential exposure and provide approved information on self-quarantine procedures, what to do if symptoms develop, and if appropriate, make referrals for testing or other resources.
  • Communicate with colleagues, supervisors and the public in a professional and empathetic manner.
  • Systematically collect and record information in a centralized database.
  • Collaborate with his/her supervisor in identifying and meeting project goals.
  • Participate in ongoing professional development and related training as well as weekly team meetings.
  • Maintain regular, frequent contact with supervisor.
  • Conduct other related duties as assigned.

 QUALIFICATIONS: 

  • Excellent interpersonal skills and ability to interact professionally, appropriately and effectively with a wide range of culturally diverse individuals during a time of crisis and distress.
  • Excellent ability to think critically and communicate both verbally and in writing in a professional and empathetic manner.
  • Computer skills are necessary and should include the ability to access and navigate various databases and software programs following training.
  • Equally comfortable working individually and within a team environment that emphasizes interdisciplinary collaboration.
  • Must be 18 years of age or older.
  • A background check is required. However, the circumstances of prior justice-involvement will be considered on a case-by-case basis and will not necessarily disqualify a candidate.
  • Present evidence of identity and authorization to work in the United States (I-9 documentation).
  • Applicants must reside, or have extensive work/lived experience, in the high economic hardship communities identified by the Chicago Department of Public Health for the purposes of this grant program.
  • Experience conducting telephone or in-person interviews, preferred.
  • Experience in clinical medicine and/or public health, preferred.
  • Customer service experience, preferred.
  • Contact tracing experience, preferred.
  • Fluency in Spanish, Polish, Mandarin, Cantonese, or additional languages highly valuable, required.

Job Summary: 

Functions as an expert in the field of Respiratory as a Respiratory Care Practitioner and performs duties that are consistent with the Hospital mission and scope of care.

·         Participates in multidisciplinary patient care planning

·         Provides active input in the management of ventilator/patient dis synchrony

·         Performs End tidal CO2 Studies with Blood gas comparisons

·         Performs therapeutic procedures of :  various secretion clearance modalities, High Flow nasal cannula therapy , speaking valve evaluations

Program Associate- CORE

 CORE is a humanitarian non-profit organization seeking a Program Associate to join its team to assist with the CORE COVID-19 Vaccine Response Team. Qualified applicants should be able to start immediately.

Responsibilities

    • Assist in field-based humanitarian response activities that support local vaccine clinics.
    • Adhere to safety protocols at all times.
    • Follow all CORE procedures and help implement at clinical sites.
    • Duties could include patient interaction (pre-screening patients for appointments, checking in/registering patients, documenting vaccine administration, observing patients post-vaccine, checking out)

Certified Nursing Assistant- CNA- Maryville Academy

    About the Certified Nursing Assistant (CNA) Position

    The Certified Nursing Assistant (CNA) assures the provision of quality health services for clients in the Children’s Healthcare Center (CHC) according to the standards and guidelines of the State of Illinois.  The CNA works under the supervision of, and takes directions from, a licensed nurse in the performance of tasks in the care of children at the CHC.  This position requires the consistent exercise of discretion and confidentiality.

    Education & Experience

    • A graduate of certified/basic nurse’s aide program;
    • Must be currently registered as a nurse’s aide in the State of Illinois;
    • Must be CPR certified;
    • Experience in a pediatric hospital is preferred;
    • Must possess a valid driver’s license.

    Knowledge, Skills & Abilities

    • Ability to communicate effectively, verbally and in writing;
    • Ability to assess, plan and evaluate patient care;
    • Able to identify sudden adverse developments in patient condition;

Bilingual Program Nurse- Maryville Academy

    The Nurse’s primary responsibility is to assure the provision of quality health services for Maryville youth according to the standards and guidelines of the Office of Refugee Resettlement (ORR).  The employee’s primary duty is work that requires advanced knowledge in the field of nursing. Position requires the consistent exercise of discretion and judgment.

    ESSENTIAL FUNCTIONS:

    • Is knowledgeable of and follows Maryville Academy Standard Medical Operating Procedures;
    • Establishes positive working relationships with the Treatment Team, program staff, and other Maryville nurses;
    • Develops and maintains lines of communication with outside contractors and consumers including ORR, physicians, dentists, therapists, and others as needed;
    • Monitors the health care delivery system of assigned sites, to include:
      • Medical clearance system;
      • Medication administration, storage, documentation and consents;
      • Delivery of prescribed patient care;
      • Patient medical record system;
      • Physicians' orders, prescription and lab work implementation in an accurate and timely manner;
      • Implements any home adaptations, as needed;
      • Oversee the medical equipment providers, as needed;
      • Ensures that medical information is up to date;
    • Coordinates with the program management staff in the discharge planning system;
    • Develops and maintains a system to provide medical consultation to case managers, Maryville staff and contracted staff;
    • Develops and provides medical workshops, groups, and CPR training for staff and clients;
    • Supervises the infection control program and quality improvement and safety program as it pertains to medical issues;
    • Communicates with the therapists, dietician(s), and psychiatrist(s) and all members of the multi-disciplinary team;
    • Participates in treatment team meetings, management team meetings and quarterly review staffing as needed;
    • Schedules client medical appointments as necessary;
    • Attends client medical appointments as indicated for the health and safety concerns of each clinet;
    • On-call 24 hours as needed, for crisis situation;
    • Participates in continued performance quality improvement;
    • Performs other work related duties as assigned. 

    MINIMUM REQUIREMENTS:

    Education/Experience:

    • Minimum of Licensed Practical Nurse. Licensed Registered Nurse, preferred;
    • Experience in pediatric nursing;
    • Physical assessment course work preferred.

    Skills /Knowledge:

    • Bilingual English/Spanish language competency required;
    • Basic computer proficiency including working knowledge of MS Word, Excel, and PowerPoint;

Laboratory Assistant- Part Time- Gift of Hope- Itasca, IL

 Assists Medical Technologists with day-to-day duties to maintain the overall operation of the laboratory. This is a part time position with some flexibility in hours, afternoon- evening schedule.
