Outreach Case Manager, Homeless Veterans’ Reintegration Program (HVRP)- The Inner Voice

The Homeless Veterans' Reintegration Program (HVRP) is an employment focused competitive grant program of the Department of Labor, Veterans' Employment and Training Service (DOL-VETS), the only federal grant to focus exclusively on competitive employment for homeless veterans. Inner Voice’s HVRP program was established in 2003 – it is the longest-running HVRP program in the state of Illinois and is currently rated as top performing in Chicago. 

The top-notch HVRP team works with the VA and other Inner Voice Veterans programs (including VA Case Management, Transition in Place) to provide wraparound services to Veterans experiencing homelessness so they can obtain/maintain employment at family sustaining wages, secure and maintain housing and enhance their well-being.

The Inner Voice, Inc. is seeking a highly qualified, dedicated individual to fill the position of Outreach Case Manager for its Homeless Veterans’ Reintegration Program.

Description of Duties; Responsible for outreach, engagement and recruiting of new participants, the Outreach Case Manager works collaboratively with the agency’s Employment Specialists to ensure participants are receiving appropriate services. Conducts intake assessments for HVRP enrollees. Assists in resume development; ensuresparticipants are enrolled with an American Jobs Center (AJC). Refers participants to local Disabled Veterans Outreach Program (DVOP) and Job Club. Works with Employment Specialists to ensure participants’ job readiness via workshops, mock interviews, etc. Provides assistance with employment applications. Tracks participant progress and other metrics such as well as referrals provided, transportation assistance, and retention services.

Primary job responsibilities/duties

• Recruit program participants through outreach and engagement at various locations throughout the city.

• Maintains working relationships with local Veteran agencies and resource centers.

• Represent the Agency at public forums and community events.

• Assist with initial participant assessments including financial, personal, and crisis assessments.

• Coordinate development of an Individualized Employment Plan (IEP) that addresses barriers, goals, and 

responsibilities; monitor its implementation through interaction with team members and participant.

• Maintain accurate and timely records of participants through detailed case noting of assessments, contacts, and 

progress within participants’ files.

• Complete referrals for services in the community and coordinate access to services.

• Assist with monthly and quarterly reports as well as data quality assurance.

Knowledge Skills and Abilities:

• Bachelor’s Degree in a related field and/or five years’ experience required.

• Proficiency with Microsoft Office products including Word, Excel, Outlook, and other relational databases 

required. Proficiency with social media preferred, as is ability to learn new software/systems. 

• Strong oral and written communication skills.

• Critical thinker and problem solver. Ability to work independently with minimal supervision.

• Excellent cross-cultural, inter-generational, collaboration and interpersonal skills.

• Experience with veterans and veterans’ services a strong plus.

• Experiencing in working with the homeless population, prior workforce experience a plus.

• High energy and patience in working with challenging participants.

• Demonstrated ability to work independently and in a team setting.

• Understanding of complex client, community, and agency issues and ability to take appropriate action.

Work Schedule: Full time (Mon-Fri 8:00am-5:00pm; some flexibility may be required.)
