Job Description

Essential Functions of the position are detailed below, and include any physical requirements 

below that.

1. Conducts intakes and assessments and creates timely individualized service plans for each participant which includes specific measurable short and long term goals and objectives. Monitors client's progress through regular visits with clients. Maintains a flexible schedule, including evenings and weekends as needed. Must be fluent in English and Spanish to provide language interpretation for clients with limited English proficiency.

2. Provides short and long-term case management services to individuals and families needing services.

3. Knowledgeable about community resources and links clients to appropriate resources. Advocates on 

behalf of clients in order to ensure access to services for which the client is eligible.

4. Works cooperatively with therapists to coordinate client services in order to help ensure the client is 

able to meet short and long term goals related to individual therapy.

5. Maintains up-to -date and accurate departmental files and databases as required by 

agency/programmatic policies and procedures, professional standards, and external requirements of all 

relevant regulatory, licensing or accrediting bodies in the course of performing job expectations.

6. Responsible for maintaining , updating and submitting routine reports.

7.Knows and complies with the laws and regulations relevant to the position and services.

8. Meets productivity expectations for client services as defined by the Department.

9. Solicits, logs, sorts, transports and places in storage in-kind donations for the program. Delivers 

needed basic household items and other resources to client's home when necessary.

10. Participates in community meetings and represents the Agency regarding services provided as well as providing accurate and timely reporting to management staff of the meeting discussions.

PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned.

X Kneel and move from sitting, bending, kneeling or standing multiple times a day.

X Push and pull objects up to 25 pounds

X Climb up and down up to 3 flights of stairs at a time.

X Lift up to 25 pounds.

Additional Requirements:

X Background check, including any program specific requirements.

X  Bi-lingual Spanish and English. May work evenings and weekends.

Education and Experience Requirements:

Minimum: Bachelor's Degree or equivalent

Relevant Experience:

Preferred: 2-4 Years

Minimum: 1-2 Years
