Bilingual case Manager for the Unaccompanied Children's Program- Maryville Academy

ESSENTIAL FUNCTIONS:

• Ensure the safety of all children within each home;

• Coordinate and handle an estimated case load of 8 at any given time, and provide

coverage on additional cases, as needed;

• Coordinate initial intake for each child;

• Actively participate in ORR staffing sessions, internal staffing, training and

organizational meetings, and Treatment Team meetings;

• Initiate and complete ORR-mandated procedures for vetting potential sponsors;

• Confirm that each child has an initial medical evaluation within 48 hours of arrival and

coordinate with nurse health interventions;

• Coordinate and assist the clinician in all required assessment procedures;

• Manage reunification process for each child;

• Keep information updated in UC portal;

• Coordinate the discharge and release of child to their sponsor;

• Respond to client emergencies within or outside work hours;

• Actively participate in ORR staffing sessions, internal staffing, training and

organizational meetings, and Treatment Team meetings;

• Fulfill ORR-mandated file requests;

• Participate in Maryville File Utilization Review and eVOLV system;

• Contribute to safety of children in the shelter as part of the larger Maryville team;

• Maintain professional relationships with

MINIMUM REQUIREMENTS:

Education/Experience:

• Bachelor’s degree in relevant behavioral/social sciences field;

• 2-4 years progressive experience in case management.

Skills /Knowledge:

• Bilingual English/Spanish language competency required;

Qualifications and Skills

Mental Health Professional- CST Program- Pilsen Wellness Center

The Community Support Team (CST) Services, funded by the Illinois Department of Human Services, Division of Mental Health, provides structured service to persons diagnosed with a severe mental illness. The program aims to reduce the frequency of hospitalizations and targets those individuals possibly isolated from supportive services. A low staff to participant ratio allows for more individualized attention and follow-up activities. Staff renders services primarily in community settings and often in the participant’s home.

We are seeking a Mental Health Professional for the CST program which is located at 2316 South Damen Avenue, Chicago, Illinois 60608.

The work schedule is Monday through Friday 9am-5pm.

Responsibilities and Duties

This individual will contribute to the implementation of the participants' service plans, specifically designed to increase the participants' skills in independent daily living, independent economic self-sufficiency and community integration/reintegration, which may include an array of services and interventions designed to provide treatment, habitation training and other integration services. Must have a valid drivers license and provide home visits.

Qualifications and Skills

This individual must have a Bachelor Degree in Psychology or a related field OR 5 years’ experience in the Mental Health Field. Familiarity with Medicaid Rule 132 desired. Bilingual in Spanish is REQUIRED.

Mental Health Professinal- HOGAR- Pilsen Wellness Center

 We are seeking a highly motivated Mental Health Professional to join our Drop In Center located at 500 East 47th Street Chicago, Illinois 60653.

The work schedule available is: Monday-Friday 8:00 am-4:00 pm.

Responsibilities and Duties

This individual is to assist the HOGAR participants to transition from long-term care facilities back in the community by helping them find appropriate independent living arrangements that will support their recovery and success. This individual will also provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual client needs. Applicant must be willing to and comfortable driving to clients and conducting home visits.

Qualifications and Skills

A Bachelor’s Degree in Counseling; Psychology; Social Work and/or 3-5 years of mental health experience or related field required. Driver's License required.

 We are seeking a highly motivated Full Time Employment Specialist to be located at our CILA Residential 2316 South Damen Avenue, Chicago, Illinois 60608.

The work schedule available is: Monday-Friday 9:00 am-5:00 pm.

Responsibilities and Duties

The Employment Specialist ensures intake and assessment of participants, instructs participants in job seeking and retention skills; develops placement plans, makes partnerships and contacts in the community helping participants to achieve employment; performs job coaching, insuring that participants find and maintain employment. Employment specialist works in an integrated manner with other Mental Health programs’ staff to provide for the supportive needs participants may present while involved in the Individual Placement & Support Program. Keeps records on services provided and prepares other required program documentation. Position involves traveling and visits to different community employers.

Qualifications and Skills

This individual must have a Bachelor’s degree in Social Work, Vocational Rehabilitation or related field. Experience in similar role preferred. Valid Driver's License Required. Bilingual (Spanish) REQUIRED.
